Your Client Has PCOS โ€” What Every PT Needs to Know

Insulin resistance, Metformin, cycle phases โ€” a practical guide for personal trainers coaching clients with PCOS.

PCOS is more common than you think

Polycystic Ovary Syndrome affects 1 in 10 women in the UK. That means if you train female clients, you almost certainly have someone with PCOS on your roster โ€” whether they have told you or not. PCOS is a hormonal condition characterised by insulin resistance, elevated androgens, and irregular ovulation. It affects weight management, energy levels, mood, and how the body responds to exercise and nutrition. Standard "eat less, move more" coaching does not work the same way for clients with PCOS.

How PCOS affects training

Insulin resistance means that carbohydrates are processed differently. High-intensity training can spike cortisol, which worsens hormonal imbalance. Recovery may be slower, especially during certain phases of an irregular cycle. Strength training is particularly beneficial for PCOS โ€” it improves insulin sensitivity independently of weight loss. Moderate intensity cardio is better tolerated than HIIT for many PCOS clients. Rest periods may need to be longer. The key is that a generic training programme designed for fat loss will not account for any of this. You need to know the condition exists and adapt accordingly.

Nutrition considerations

For PCOS, nutrition is arguably more important than training. Lower glycaemic index carbohydrates help manage insulin spikes. Higher protein intake supports satiety and muscle retention. Anti-inflammatory foods can help manage systemic inflammation. If your client is on Metformin โ€” the most commonly prescribed medication for PCOS โ€” you need to know that it reduces B12 absorption over time. They may need B12 supplementation. Metformin should be taken with food to reduce side effects. And the timing of carbohydrate intake relative to medication matters. Supplements that research suggests may help include Inositol (particularly myo-inositol and D-chiro-inositol), Vitamin D, Omega-3, and Berberine. But these should be discussed with their GP, not prescribed by a PT.

Cycle phase matters โ€” when it exists

Many women with PCOS have irregular cycles, which makes cycle-phase coaching more complex. When a cycle is present, the same principles apply as for any female client โ€” the follicular phase supports higher intensity training, while the luteal phase may require reduced intensity. The difference is that cycle length can vary significantly, and some clients may not ovulate regularly. Tracking helps identify patterns even when cycles are irregular.

How most PT software handles this
